Pre-Registration:
The Registration Department will make an attempt to contact you before surgery to confirm demographic (personal information), insurance information, and hopefully answer any questions you may have prior to your surgery. We will also notify you of your estimated co-pay, co-insurance, and/or deductible due the day of service.
A Pre-Operative Nurse will call you prior to your surgery to collect information about your health to expedite your admission process. If, however, you are not contacted by someone from our facility by 12:00 p.m. the day prior to your surgery, please contact us at 859-276-2525 and ask to speak with both pre-registration and the pre-operative nurse.
Advance Directive/Living Will
An advance directive states, in writing, your choices about medical care, or names someone to make medical decisions for you if you become unable to speak for yourself. It is called an “advance directive” because it is signed in advance to let your family and doctor know your wishes regarding medication treatment. Kentucky law recognizes many types of advance directives, including living will directives, healthcare surrogate designations, durable powers of attorney and mental health treatment directives.
You are not required to have an advance directive to have surgery at the Lexington Surgery Center. The Lexington Surgery Center is required by law to inquire if you have an advance directive and if you do not, provide you with information concerning advance directives if requested.
If you have an advance directive we ask that you bring a copy with you the day of surgery and inform the nurse you have the directive so a copy may be placed in the medical record. When a patient or their representative indicates the patient has executed an advance directive, the facility will inform the patient or their representative an advance directive will NOT be honored during the patient’s episode of care at the Lexington Surgery Center. When the patient presents an advance directive, the facility will inform the patient of the facility policy and a copy of the advance directive will be placed on the medical record for reference in the event the patient is transferred to an acute care facility.
NOTICE TO PATIENTS
Medicare regulations requires all health facilities which are licensed under the provisions of Centers for Medicare and Medicaid Services 42 CFR Part 416 {CMA-3887-P} RIN 0938-AL80 to inform the patient of the “Patient’s Bill Of Rights” and the physician ownership in the facility. This information will be given to you at the time your procedure is scheduled and also will be posted in the waiting areas of the Lexington Surgery Center.
If you have any suggestions, concerns or grievances which arise out of these rights the Lexington Surgery Center has the following process in place to address and seek resolution:
